Our Primary Care Network

The new GP contract was published in early 2019 as part of the NHS Long Term Plan. A key element of the new contract is the development of Primary Care Networks (PCNs) which bring GP practices together with other local services – such as community, mental health, social care, and the voluntary sector – to look after local populations. PCNs help to join up services at a local level, focusing on the specific needs of these local populations, with patients still accessing routine GP appointments as they do now.

Miles Platting, Newton Heath & Moston PCN

Individual GP practices have been establishing and joining PCNs covering populations of between 30,000 to 50,000

Miles Platting, Newton Heath & Moston PCN is made up of 7 different surgeries. They are:

  • Droylesden Road Family Practice
  • Hazeldene Medical Centre
  • Newton Heath Medical Centre
  • Simpson Medical Practice
  • St George’s Medical Centre
  • Victoria Mill Medical Practice
  • Whitley Road Medical Centre

Extended access

Our surgery teams are working closely with each other, enjoying the ability to share expertise and resources, to develop new services. Because we are part of a PCN we can take advantage of additional staff roles that are now available to support all our patients which is usually located at Newton Heath Medical Centre. These additional roles help us to provide the right care at the right time, from the right professional. The following video produced by NHS East of England helps to explain these additional roles:

Integrated Neighbourhood Team

Our integrated neighbourhood team brings together community health and social care staff, so they work together around your needs. Our team has a leadership team that guide their day-to-day work and looks at the longer-term things we need to do to help support health and wellbeing in the neighbourhood.
